Contact forms Shopify stores can use start with the built-in form on your store’s contact page. To add one, create a page under Online Store > Pages and pick the contact theme template. Every submission is emailed to your sender email address and checked by Shopify’s spam filters. It arrives as free text with no order context unless you add fields or send it to a helpdesk.
How do contact forms in Shopify work?
Contact forms in Shopify are a built-in theme feature. A shopper fills in the form, Shopify screens the message with its spam filters and emails it to your store’s sender email address. According to Shopify’s contact page guide, the sender address is the single destination. The guide describes the submission as an email to that address and documents no ticketing or order lookup.

Shopify’s reference theme, Dawn, ships a form with four inputs: name, email, phone and comment, per its contact-form source. Email is the only one required to submit, according to shopify.dev.
| Item | How the built-in form behaves |
|---|---|
| Default fields (Dawn) | Name, email, phone, comment |
| Required input | Email only (contact[email]) |
| Destination | Your store’s sender email address |
| Spam handling | Shopify’s filters analyse the message body; flagged mail gets a [SPAM] prefix |
| Subject line | Fixed; Shopify says you can’t change it |
| Where you change the address | Notifications settings page |
The sources are the Shopify pages linked above, fetched on 12 October 2026. Our view: the built-in form is an intake form, not a support system. It collects a message and, as far as Shopify documents, leaves you to find the order.
How do you add a contact form in Shopify?
You add a contact form by creating a page that uses the contact theme template. It takes six steps and no app. Shopify’s own instructions follow the same path, and the steps below are in our words.
- In your admin, open Online Store > Pages.
- Choose Add page.
- Enter a title, such as “Contact us”.
- Optionally add intro content above the form.
- In the Theme template drop-down, select “contact”.
- Save.
There is a second route. Shopify says you can add a contact form to any page with the Contact form section, but only if your theme supports it, so check with your theme developer. In Dawn and other themes, the exact field names can differ, so check your own theme.
To change where submissions go, open the Notifications settings page and edit the sender email. If you use a third-party domain, it may need authenticating.
One piece of advice from us: link the contact page from the footer and from the order status flow, and put a short line above the form pointing shoppers to self-service for order status. Our order status page shows how “where is my order” tickets get resolved end to end.
What types of contact form can a Shopify store use?
A Shopify store can use six kinds of contact form: the built-in page template, the Contact form section, the built-in form with custom fields, an embedded form builder, a helpdesk’s own form, and Shopify Forms. Only the first five collect support requests. The table gives one example each.
| Type | One named example | What it does |
|---|---|---|
| Contact page template | Theme template “contact” | Emails the message to your sender address |
| Contact form section | Theme’s “Contact form” section | Adds the same form to any page, if the theme supports it |
| Built-in form, custom fields | Your own contact[...] inputs | Same destination, richer content |
| Embedded form builder | Wufoo or Jotform, which Shopify suggests | Sends submissions elsewhere than the sender address |
| Helpdesk form | Gorgias Contact Form | Turns submissions into tickets on a “Contact Form” channel |
| Shopify Forms | Shopify Forms app | Popup or inline sign-up forms. Not a support form |
Shopify Forms is the one to watch for. Per Shopify’s documentation, it grows your subscriber list and builds customer segments, which makes it a marketing tool. Shopify’s June 2025 change (changelog) applies to that app, not to the theme contact form, so do not read it as a fix for support intake.
We deliberately skip an app roster here. The type matters more than the vendor, because it decides where the message lands.
How do you add custom fields to a Shopify contact form?
You can add custom fields without an app. Edit your theme’s contact form section and give each input a name in the form contact[your_label]. Per shopify.dev, those labels appear in the notification email and must be unique. Add required="required" to make a field mandatory.
That corrects a common claim that the built-in form has no custom fields. It has them, in code. A minimal example:
<label for="ContactOrder">Order number</label>
<input type="text" id="ContactOrder" name="contact[order_number]" required="required">
<label for="ContactReason">Reason</label>
<select id="ContactReason" name="contact[reason]" required="required">
<option>Where is my order</option>
<option>Return or exchange</option>
<option>Damaged item</option>
<option>Product question</option>
<option>Other</option>
</select>
Our recommended fields are an order number, a reason drop-down and, if you can, an optional attachment through an app or a helpdesk form. The reason is simple. Structured fields are what a helpdesk rule or an AI agent can route and resolve on. A free-text message that says “my parcel hasn’t come” forces someone to ask which order.
Where should Shopify contact form submissions go?
Submissions should go into your helpdesk as tickets, not into a shared inbox. The built-in form can only send to the one sender email address, so either forward that mailbox into your helpdesk or use the helpdesk’s own form. A shared inbox has no owner, no status and no history.
Zendesk. Zendesk creates tickets from email forwarded from an external address once forwarding is verified. Its help article recommends server-level forwarding and warns that manually forwarding a message from an external support address results in a suspended ticket.
Gorgias. Gorgias is a strong e-commerce helpdesk, and its Gorgias Contact Form creates tickets on a “Contact Form” channel, with built-in spam protection and multiple attachments, according to its doc (fetched 12 October 2026).
A practical check, as advice rather than a stated Shopify behaviour: submit a test message and confirm that the ticket requester is the shopper, not your own or Shopify’s address, before you build any rules on it.
What are the limits of the built-in Shopify contact form?
The built-in form has a fixed subject line, a [SPAM] prefix on flagged messages, a hCaptcha setting that conflicts with other CAPTCHA code, one destination address and, in our view, no order context. Each gap is one the person answering has to close by hand. The table pairs each limit with the usual fix.
| Limit | Source | Usual fix |
|---|---|---|
| You can’t change the subject line | Shopify guide | Add a reason field and filter on the body, or use a helpdesk form |
Flagged submissions get a [SPAM] prefix; filters analyse contact[body] | Same guide | Check your filters before assuming the form is broken |
| hCaptcha is on by default and isn’t compatible with custom code or apps that add CAPTCHA | Shopify Preferences | Remove the conflicting CAPTCHA app, or toggle under Online Store > Preferences > Spam protection |
| One destination address | Same guide | Forward the mailbox, or use a helpdesk form |
| Free text, no order context (our view: Shopify documents no order lookup) | Our view | Custom fields, then an agent that reads the order |
Sources fetched on 12 October 2026. To stop spam getting through, the control Shopify documents is the hCaptcha toggle under Online Store > Preferences > Spam protection; leave it on unless it conflicts with other CAPTCHA code. If your emails arrive marked [SPAM], that is the filter at work on the message body, not a broken form. Check what the shopper actually wrote first.
Does a contact form reduce support tickets?
No. A contact form changes how tickets arrive, not how many get resolved. Gorgias’s doc says its Contact Form can run Order Management and Flows before submission, for Automate subscribers. That is sensible deflection, but every ticket that does get submitted still needs an answer afterwards. Our guide to AI agents covers resolution after submit, and the Shopify Magic, Magento and customer support guides cover the wider picture.
Where does Engaige fit?
Engaige (that’s us) isn’t a form builder: it doesn’t add or style a contact form. It is the e-commerce-native AI support agent that plugs into the helpdesk you already run and works on the form tickets that reach your helpdesk. At MR MARVIS, a Shopify Plus brand, it resolves over 60% of the conversations it is involved in. Keep your helpdesk. Replace the AI.

Take a “where is my order” ticket. On Shopify, Engaige combines live Admin API data (orders, fulfilment state, customers) with the content from your storefront, then answers from the real order, not with a canned acknowledgement: it drafts the reply in Agent Assist, or sends it once that ticket type is autonomous. The actions it can take are listed on our Shopify integration page.
Hi, where is my order? It's been eight days and the tracking hasn't moved.

Your parcel went back to the depot after three delivery attempts, so it will not arrive. I have booked a reshipment to the address you confirmed, arriving Thursday.
Every answer and action shows why, with the policy and source it used, so you can see where an answer came from and fix any gap. Engaige AI learns from the difference between what your team sends and what the agent drafted, proposes improvements with their reasoning, and you approve what goes live.
Within the AI we can see the reasoning, and how it comes to a response. That shows us where information is missing or unclear, and helps us improve our workflow.
You go live in Agent Assist in hours. Engaige is text-first by design: email, chat, WhatsApp, SMS and social DMs, no voice AI. A contact form is one more email-type intake.
Frequently asked questions
Does Shopify have a built-in contact form?
Yes. Create a page with the contact theme template and Shopify gives you a form that emails every submission to your store’s sender email address.
How do I add a contact us form in Shopify?
Go to Online Store > Pages > Add page, enter a title, optionally add intro content, select “contact” in the Theme template drop-down and save. If your theme supports it, the Contact form section adds the same form to any page.
Where do Shopify contact form submissions go?
To your store’s sender email address, which you change on the Notifications settings page. To get tickets rather than emails, forward that mailbox into your helpdesk or use the helpdesk’s own form.
Can I add custom fields to the Shopify contact form without an app?
Yes. Name each input contact[your_label] in the theme’s contact form code, keep labels unique and add required="required" where a field is mandatory.
Why are my Shopify contact form emails marked [SPAM]?
Shopify’s spam filters analyse the message body and prefix flagged submissions with [SPAM]. hCaptcha is also on by default and conflicts with custom code or apps that add their own CAPTCHA.
Can I change the subject line of Shopify contact form emails?
No. Shopify says you can’t change it. Add a reason field, or use a helpdesk form that sets its own subject.
Should Shopify contact form messages go to a helpdesk?
Yes, as tickets. A shared inbox has no status or history. With a helpdesk, an AI agent such as Engaige can read the order and work the ticket inside the tool your team already uses.
Can an AI agent answer Shopify contact form tickets?
Yes, once the submission lands as a ticket in your helpdesk. Engaige reads the live Shopify order and drafts or sends a reply from it, depending on whether that ticket type is autonomous yet, and you approve what it learns.
